The care manager plays a pivotal role in the Collaborative Care Model (CoCM). They engage patients, provide behavioral interventions, coordinate information between members of the team, with other providers, and with family members, as well as participate in a unique data-driven process with the psychiatric consultant to make sure patients get the most effective care possible. This course assists care managers in acquiring the knowledge, skills, and attitudes that are crucial for their role in the primary care setting.

Course Agenda

This training sequence rests upon a working knowledge of foundational concepts of the CoCM, including the rationale, evidence base, basic principles, and team roles.  Topics include:

Section 1

  • Patient Engagement (21 min)
  • Introducing the CoCM to Patients (5 min)

Section 2

  • Psychiatric Medication Support (20 min)

Section 3

  • Collaborative Care Fundamentals: The Registry (12 min)
  • Caseload Management (25 min)
  • Caseload Review with The Psychiatric Consultant (5 min)
  • Case Presentation Skills (7 min)

Learning Objectives

  • Review the foundational concepts of the Collaborative Care Model;
  • Develop an understanding and skills needed for the care manager’s role in the primary care setting; and
  • Gain a shared understanding of the new roles required by all the members of the Collaborative Care team.

How to Earn Credit

Participants who wish to earn a certificate of completion may do so by completing all sections of the course including the evaluation. After evaluating the program, course participants will be provided with an opportunity to claim and print a certificate of completion showing the date and participation hours earned. CME credit for this training is not available.

The American Psychiatric Association (APA) is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians. The APA does not designate this enduring activity for AMA PRA Category 1 Credit™.
