Post-stroke depression (PSD) occurs in a significant number of stroke patients and can lead to greater disability and increased mortality. Recent meta-analyses have demonstrated the efficacy of antidepressants in treating and preventing PSD. In addition, there has been progress in understanding the pathophysiology of PSD, including inflammatory processes, genetic and epigenetic variations, white matter disease, and alterations in neurotransmission. Future research should focus on identifying additional aspects of the mechanism of PSD in order to develop more specific therapeutic interventions. There is also a need to determine the mechanisms of increased mortality following PSD and elucidate how antidepressants enhance recovery after stroke.
