Video Summary
Dr. James Bacoff presented a broad overview of ADHD across the lifespan, emphasizing that it is a real, brain-based, highly heritable disorder with significant personal, social, and economic burden. He noted that ADHD affects about 3–10% of school-age children and 3–5% of adults worldwide, with many cases continuing into adulthood. He highlighted common comorbidities and risks, including oppositional behavior, depression, anxiety, substance use, school and job difficulties, relationship problems, and earlier mortality.

He reviewed the evolution of thinking about ADHD, from outdated ideas about bad parenting or simple developmental delay to current evidence showing strong genetic influence and brain network differences. Environmental and prenatal factors can worsen symptoms, but do not cause the disorder on their own.

For assessment, he stressed the importance of a careful developmental history, collateral information when possible, symptom ratings as screening tools, and a full medical evaluation. He emphasized that no blood test, brain scan, or neuropsychological test is diagnostic for ADHD, though testing can help with planning.

Treatment recommendations included psychoeducation, school or workplace accommodations, parenting supports, CBT, and medications. Stimulants remain the most effective treatment, with methylphenidate and amphetamine options chosen based on age, tolerability, cost, and response. He also discussed non-stimulants, side effects, dose titration, monitoring, and managing ADHD with comorbid anxiety, depression, bipolar disorder, substance use, autism, or trauma.
